Box 5121–00200, Nairobi in the Republic of Kenya, formerly known as Berly Celine Achieng, formally and absolutely renounced and abandoned the use of her former name Berly Celine Achieng, and in lieu thereof assumed and adopted the name Berly Achieng Agutu, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Berly Achieng Agutu only.
