Box 90469–80100, Mombasa in the Republic of Kenya, on behalf of Jacey Gift Otingo (minor), formerly known as Jacey Scola Otingo, formally and absolutely renounced and abandoned the use of her former name Jacey Scola Otingo and in lieu thereof assumed and adopted the name Jacey Gift Otingo, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Jacey Gift Otingo only.
