Box 46967–00100, Nairobi in the Republic of Kenya, on behalf of Chris Junior Mutie (minor), formerly known as Chris Junior Kirimi, formally and absolutely renounced and abandoned the use of his former name Chris Junior Kirimi and in lieu thereof assumed and adopted the name Chris Junior Mutie, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Chris Junior Mutie only.
