Box 50005–00101, Nairobi in the Republic of Kenya, on behalf of Hannah Waruguru Kiarie (minor), formerly known as Hanah Waruguru Kiarie, formally and absolutely renounced and abandoned the use of her former name Hanah Waruguru Kiarie and in lieu thereof assumed and adopted the name Hannah Waruguru Kiarie, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Hannah Waruguru Kiarie only.
