Box 12428–00400, Nairobi in the Republic of Kenya, formerly known as Jaclyne Rhoda Akinyi Agengo, formally and absolutely renounced and abandoned the use of her former name Jaclyne Rhoda Akinyi Agengo and in lieu thereof assumed and adopted the name Jazlyne Dzuya Agengo, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Jazlyne Dzuya Agengo only.
