Box 104065– 00101, Nairobi in the Republic of Kenya, on behalf of Skylar Imani Wendo (minor), formerly known as Skylar Imani Wendo Njoroge, formally and absolutely renounced and abandoned the use of her former name Skylar Imani Wendo Njoroge, and in lieu thereof assumed and adopted the name Skylar Imani Wendo, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Skylar Imani Wendo only.
