Box 87171–80100, Mombasa in the Republic of Kenya, on behalf of Jeremy Raha Ephraim (minor), formerly known as Jeremy Aliaro Liboyi, formally and absolutely renounced and abandoned the use of his former name Jeremy Aliaro Liboyi, and in lieu thereof assumed and adopted the name Jeremy Raha Ephraim, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Jeremy Raha Ephraim only.
