Box 265–00621, Nairobi in the Republic of Kenya, formerly known as Barbara Damaris Wambui Mungai, formally and absolutely renounced and abandoned the use of her former name Barbara Damaris Wambui Mungai and in lieu thereof assumed and adopted the name Barbara Damaris Wambui Koinange, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Barbara Damaris Wambui Koinange only.
