Box 1–80400, Ukunda in the Republic of Kenya, formerly known as Rebbeca Waithira Wanja, formally and absolutely renounced and abandoned the use of my former name Rebbeca Waithira Wanja, and in lieu thereof assumed and adopted the name Rebbeca Waithira Beutner, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Rebbeca Waithira Beutner only.
