Box 4285– 00200, Nairobi in the Republic of Kenya, formerly known as Charlene Janira Orendo, formally and absolutely renounced and abandoned the use of her former name Charlene Janira Orendo, and in lieu thereof assumed and adopted the name Charlene Janira, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Charlene Janira only.
