Box 3720–80100, Mombasa in the Republic of Kenya, formerly known as Nabila Anwar Mbarak, formally and absolutely renounced and abandoned the use of her former name Nabila Anwar Mbarak, and in lieu thereof assumed and adopted the name Nabila Anwar Hamada, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Nabila Anwar Hamada only.
