Box 138, Bunyore in the Republic of Kenya, formerly known as Anita Shally alias Anita Shally Elias, formally and absolutely renounced and abandoned the use of her former name Anita Shally alias Anita Shally Elias, and in lieu thereof assumed and adopted the name Anitah Dickson Mmeli, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Anitah Dickson Mmeli only.
