Box 2112–20100, Nakuru in the Republic of Kenya, formerly known as Sarah Kagure Gikonyo alias Sarah Kagure Kinuthia alias Jane Njeri Ndungu, formally and absolutely renounced and abandoned the use of my former name Sarah Kagure Gikonyo alias Sarah Kagure Kinuthia alias Jane Njeri Ndungu, and in lieu thereof assumed and adopted the name Jane Njeri Larcombe, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Jane Njeri Larcombe only.
