Box 58543– 00200, Nairobi, formerly known as Sylvia Wanjiru Kabiro, formally and absolutely renounced and abandoned the use of her former name Sylvia Wanjiru Kabiro, and in lieu thereof assumed and adopted the name Sylvia Wanjiru Madsen, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Sylvia Wanjiru Madsen only.
