Box 888–20300, Nyahururu, in the Republic of Kenya, formerly known as Mary Waruguru Kariuki alias Mary Waitherero Kariuki, formally and absolutely renounced and abandoned the use of my former name Mary Waruguru Kariuki alias Mary Waitherero Kariuki, and in lieu thereof assumed and adopted the name Mary Herren Nduati, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Mary Herren Nduati, only.
