Box 81075, Mombasa in the Republic of Kenya, formerly known as Bansi Govind Ravji Vekaria, formally and absolutely renounced and abandoned the use of her former name Bansi Govind Ravji Vekaria, and in lieu thereof assumed and adopted the name Bansi Govind Ravji Lalji, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Bansi Govind Ravji Lalji only. GOVIND RAVJI LALJI, as the father and natural guardian of Bansi Govind Ravji Lalji, MR/9959064 formerly known as Bansi Govind Ravji Vekaria.
