Box 38-70101, Hola in the Republic of Kenya, formerly known as Faith Marta Kiriago, formally and absolutely renounced and abandoned the use of my former names Faith Marta Kiriago and in lieu thereof assumed and adopted the name Faith Marta Yesse, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Faith Marta Yesse only.
