NOTICE is given that by a deed poll dated 6th December, 2011, duly executed and registered in the Registry of Documents at Mombasa as Presentation No. 372, in Volume B. 13, Folio 943/4821, File No. 1637, by me, Hetal Nirmal Gurjar, of P.O. Box 80101, Mombasa in the Republic of Kenya, formerly known as Harshila Nirmal Gurjar, formally and absolutely renounced and abandoned the use of my former name Harshila Nirmal Gurjar, and in lieu thereof assumed and adopted the name Hetal Nirmal Gurjar, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Hetal Nirmal Gurjar only. HETAL NIRMAL GURJAR, MR8843012 formerly known as Harshila Nirmal Gurjar.
