Box 1079–00517, Nairobi in the Republic of Kenya, formerly known as Elizabeth Diana Wanjiru Wanjohi, formally and absolutely renounced and abandoned the use of her former name Elizabeth Diana Wanjiru Wanjohi, and in lieu thereof assumed and adopted the name Lisa Wanjiru Wanjohi, for all purposes and authorize and request all persons at all times to designate, describe and address her by her assumed name Lisa Wanjiru Wanjohi only.
