Box 46861-00100, Nairobi in the Republic of Kenya, formerly known as Eunice Muthoni Karoki, formally and absolutely renounced and abandoned the use of my former names Eunice Muthoni Karoki and in lieu thereof assumed and adopted the name Eunice Muthoni Kumunga, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Eunice Muthoni Kumunga.
