Box, 87349–80100, Mombasa in the Republic of Kenya, formerly known as Irene Barongo Gitaa, formally and absolutely renounced and abandoned the use of my former name Irene Barongo Gitaa, and in lieu thereof assumed and adopted the name Irene Ishmael Gitaa, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Irene Ishmael Gitaa only.
