Box 34, Charage-ini in the Republic of Kenya, formerly known as Joseph Kayai Mwaniki, formally and absolutely renounced and abandoned the use of my former name Joseph Kayai Mwaniki, and in lieu thereof assumed and adopted the name Joseph Kayai Machigo, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Joseph Kayai Machigo only.
