Box 349, Bungoma in the Republic of Kenya, formerly known as Emmanuel Barasa Nasongo Siundu, formally and absolutely renounced and abandoned the use of my former name Emmanuel Barasa Nasongo Siundu, and in lieu thereof assumed and adopted the name Emanuel Barasa Nasongo, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Emanuel Barasa Nasongo only.
