NOTICE is given that the first statutory meeting of Samburu Central District Alcoholic Drinks Regulatory meeting will be held on 25th July, 2011, in District Commissioner‘s boardroom at 10.00 a.m. Applications to be considered at the meeting whether new, renewal, transfers, removal or conversion of licences should be submitted on prescribed Form I (R. 4 (2) or Form 4 (R. 4 (5) in case of transfers or removal in triplicate addressed to the Chairman, District Alcoholic Drinks Regulatory Committee, Samburu Central, so as to reach on or before 1st July, 2011. The following documents shall be mandatory: Application Form I, Identification documents (ID or possport for individuals) or certificate of registration or incorporation for corporate bodies, copy of PIN certificate for applicants and Directors for corporate bodies, VAT certificate from KRA, public health certificate, names of persons to maage , superintend or conduct day to day business or the premises (in accordance with section 2 (2) of the Act, a sketch map describing the location of the premises, physical addresses indicating names of the area where the premises are located, names of building where premises are situated and plot No. where applicable. All applicants must pay applicable fee of Kenya shillings one thousand (KSh. 1,000) only. Payable to NACADA Alcohlic Drink Control Fund, Kenya Commercial Bank, Account No. 1125676671, Milimani Branch, Nairobi or Co-operative Bank, Account No. 01141134561900, Ukulima Branch, Nairobi. Applicants for renewal must attach copies of their current licence and send their applications by registered post or deliver them by hand.
