Box 872, Kilifi in the Republic of Kenya, formerly known as Caroline Karisa Charo Koba, formally and absolutely renounced and abandoned the use of my former name Caroline Karisa Charo Koba, and in lieu thereof assumed and adopted the name Caroline Kwicha Baya, for all purposes and authorizes and request all persons at all times to designate, describe and address me by my assumed name Caroline Kwicha Baya only.
