Box 43190–00100, Nairobi in the Republic of Kenya, formerly known as Sammy Kipkorir Chemjor, formally and absolutely renounced and abandoned the use of my former name Sammy Kipkorir Chemjor, and in lieu thereof assumed and adopted the name Sammy Kipkorir Wendot, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Sammy Kipkorir Wendot only.
