Box 82991, Mombasa in the Republic of Kenya, formerly known as Dhiraj Premji Naran Shivji, formally and absolutely renounced and abandoned the use of my former name Dhiraj Premji Naran Shivji, and in lieu thereof assumed and adopted the name Dhiraj Premji Halai, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Dhiraj Premji Halai only.
