NOTICE is given that by a deed poll dated 29th March, 2011, duly executed and registered in the Registry of Documents at Mombasa as Presentation No. 101 in Volume B–13, Folio 905, File No. 1637, by me Arvindkumar Lalji Patel as the father and natural guardian of Khushbu Arvindkumar Patel, of P.O. Box 83320, Mombasa in the Republic of Kenya, formerly known as Khushbuben Arvindkumar Patel, formally and absolutely renounced and abandoned the use of my minor daughter‘s former name Khushbuben Arvindkumar Patel, and in lieu thereof assumed and adopted the name Khushbu Arvindkumar Patel, for all purposes and authorizes and requests all persons at all times to designate, describe and address my said minor daughter by her assumed name Khushbu Arvindkumar Patel only. ARVIND KUMAR LALJI PATEL, as the father and natural guardian of Khushbu Arvindkumar Patel, MR1944382 formerly known as Khushbuben Arvindkumar Patel.
