Box 30213–00100, Nairobi in the Republic of Kenya, formerly known as Faith Wambui Macharia, formally and absolutely renounced and abandoned the use of my former name as Faith Wambui Macharia, and in lieu thereof assumed and adopted the name Faith Wambui Ngotho, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Faith Wambui Ngotho only.
