Box 5528– 30100, Eldoret in the Republic of Kenya, formerly known as Richard Ogwaro J. Muhomah alias Fredrick Richard Muhomah alias Fredrick Richard Muhoma alias Richard Muhoma alias Richard Muhomah alias Richard Ogwaro Josiah Muhomah, formally and absolutely renounced and abandoned the use of his former name Richard Ogwaro J. Muhomah alias Fredrick Richard Muhomah alias Fredrick Richard Muhoma alias Richard Muhoma alias Richard Muhomah alias Richard Ogwaro Josiah Muhomah, and in lieu thereof assumed and adopted the name Fredrick Richard Ogwaro Muhomah, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Fredrick Richard Ogwaro Muhomah only.
