Box 9442–00200, Nairobi in the Republic of Kenya, formerly known as Samuel Githinji Nyakairungi, formally and absolutely renounced and abandoned the use of me former name Samuel Githinji Nyakairungi, and in lieu thereof assumed and adopted the name Samuel Githinji, for all purposes and authorize and request all persons at all times to designate, describe and address me by my assumed name Samuel Githinji only.
