Box 71.13-00300, Nairobi in the Republic of Kenya, formerly known as Jacktone Caxtone alias Okeyo Caxtone Jack alias Jack Odoyo Ochieng, formally and absolutely renounced and abandoned the use of his former name Jacktone Caxtone alias Okeyo Caxtone Jack alias Jack Odoyo Ochieng, and in lieu thereof assumed and adopted the name Jack Caxtone Okeyo, for all purposes and authorizes and requests all persons at all times to designate, describe and address him by his assumed name Jack Caxtone Okeyo only.
