Box 222–80300, Voi in the Republic of Kenya, on behalf of Janelle Doret Sanguli Mwawongo (minor), formerly known as Janelle Doret Sanguli Mwakughu, formally and absolutely renounced and abandoned the use of her former name Janelle Doret Sanguli Mwakughu , and in lieu thereof assumed and adopted the name Janelle Doret Sanguli Mwawongo, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Janelle Doret Sanguli Mwawongo only.
