Box 315–00217, Limuru in the Republic of Kenya, formerly known as Miriam Wanjiku Kuria, formally and absolutely renounced and abandoned the use of my former name Miriam Wanjiku Kuria, and in lieu thereof assumed and adopted the name Wanjiku Kuria Mwangi, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name, Wanjiku Kuria Mwangi only.
