Box 81911–80100, Mombasa in the Republic of Kenya, formerly known as Kubranhussein Farida Zoeb Mulla, formally and absolutely renounced and abandoned the use of her former name Kubranhussein Farida Zoeb Mulla, and in lieu thereof assumed and adopted the name Farida Zoeb Ezzi, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Farida Zoeb Ezzi only.
