Box 59274– 00200, Nairobi in the Republic of Kenya, formerly known as Odera A. Beatrice, formally and absolutely renounced and abandoned the use of her former name Odera A. Beatrice and in lieu thereof assumed and adopted the name Beatrice Odera, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Beatrice Odera only.
