Box 67839–00200, Nairobi in the Republic of Kenya, on behalf of Eliana Hadassah Mwikali Muange (minor), formerly known as Hadassah Muthoni Mwangi, formally and absolutely renounced and abandoned the use of her former name Hadassah Muthoni Mwangi and in lieu thereof assumed and adopted the name Eliana Hadassah Mwikali Muange, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Eliana Hadassah Mwikali Muange only.
