Box 79248– 00200, Nairobi in the Republic of Kenya, formerly known as Tabitha Dorcas Njeri Keinamma, formally and absolutely renounced and abandoned the use of her former name Tabitha Dorcas Njeri Keinamma and in lieu thereof assumed and adopted the name Tabby Njeri Keinamma, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Tabby Njeri Keinamma only.
