Box 99646–80107, Mombasa in the Republic of Kenya, formerly known as Eve Adhiambo Oduk, formally and absolutely renounced and abandoned the use of her former name Eve Adhiambo Oduk, and in lieu thereof assumed and adopted the name Eve Adhiambo Kongere, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Eve Adhiambo Kongere only.
