Box 40776– 00100, Nairobi in the Republic of Kenya, formerly known as N. J. Kageni Phinehas Nyaga, formally and absolutely renounced and abandoned the use of her former name N. J. Kageni Phinehas Nyaga, and in lieu thereof assumed and adopted the name Nelly Kageni Nyaga, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Nelly Kageni Nyaga only.
