Box 62852– 00511, Ngong in the Republic of Kenya, formerly known as Ann Wangui Muthee, formally and absolutely renounced and abandoned the use of her former name Ann Wangui Muthee, and in lieu thereof assumed and adopted the name Ann Loise Wangui, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Ann Wangui Muthee only.
