Box 10–10400, Nanyuki in the Republic of Kenya, formerly known as Cardie Wanjiru Wanjiku, formally and absolutely renounced and abandoned the use of her former name Cardie Wanjiru Wanjiku and in lieu thereof assumed and adopted the name Cardie Vilmah Hanif, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Cardie Vilmah Hanif only.
