Box 4045– 00200, Nairobi in the Republic of Kenya, formerly known as Stephanie Keza Mpyisi Omwanza, formally and absolutely renounced and abandoned the use of her former name Stephanie Keza Mpyisi Omwanza and in lieu thereof assumed and adopted the name Keza Mpyisi Omwanza, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Keza Mpyisi Omwanza only.
