Box 786– 00516, Nairobi in the Republic of Kenya, formerly known as Ruth Wambui Kiama, formally and absolutely renounced and abandoned the use of her former name Ruth Wambui Kiama and in lieu thereof assumed and adopted the name Ruth Wambui Muigai, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Ruth Wambui Muigai only.
