Box 32293–00600, Nairobi in the Republic of Kenya, formerly known as Christabel Achieng Omondi alias Christabel Joan Achieng Bodi alias Christabel John Achieng, formally and absolutely renounced and abandoned the use of her former name Christabel Achieng Omondi alias Christabel Joan Achieng Bodi alias Christabel John Achieng and in lieu thereof assumed and adopted the name Christabel Achieng Bodi, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Christabel Achieng Bodi only.
