Box 74–80305, Mwatate in the Republic of Kenya, formerly known as Peter Henry Hunderson Mazozo, formally and absolutely renounced and abandoned the use of my former name Peter Henry Hunderson Mazozo, and in lieu thereof assumed and adopted the name Peter Mazozo, for all purposes and authorizes and requests all persons at all times to designate, describe and address me by my assumed name Peter Mazozo only.
