Box 13– 00216, Kiambu in the Republic of Kenya, formerly known as Cecily Nyachira Kahara, formally and absolutely renounced and abandoned the use of her former name Cecily Nyachira Kahara, and in lieu thereof assumed and adopted the name Cecily Nyachira Ngigi, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Cecily Nyachira Ngigi only.
