Box 5436–00506, Nairobi in the Republic of Kenya, formerly known as Caroline Mwende, formally and absolutely renounced and abandoned the use of her former name as Caroline Mwende and in lieu thereof assumed and adopted the name Caroline Mwende Zeyhle, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Caroline Mwende Zeyhle only.
