Box 174–00507, Nairobi in the Republic of Kenya, on behalf of Alma Njeri Kiambati (minor), formerly known as Alma Muthoni, formally and absolutely renounced and abandoned the use of her former name Alma Muthoni, and in lieu thereof assumed and adopted the name Alma Njeri Kiambati, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Alma Njeri Kiambati only.
