Box 53743–00200, Nairobi in the Republic of Kenya, formerly known as Beverly Sarah Nasimiyu, formally and absolutely renounced and abandoned the use of her former name Beverly Sarah Nasimiyu, and in lieu thereof assumed and adopted the name Simiyu Beverly Nasimiyu, for all purposes and authorizes and requests all persons at all times to designate, describe and address her by her assumed name Simiyu Beverly Nasimiyu only.
